Mercyhurst College Student Health Insurance Policy
Enrollment:
Any undergraduate or graduate student of Mercyhurst College and their dependants can enroll in the Student Health Plan. Payment of the premium must accompany the enrollment form at the time of submission. For payment options please see Premium Rates and Deductibles.
Enrollment Deadlines:
	Athletes:
	All waiver forms and accompanying documentation must be submitted no later than July 31, 2010. Late Enrollments/waivers with an effective date of August 1, 2010 will be accepted until August 15, 2010
	International Students Students:
	All waiver forms and accompanying documentation must be submitted no later than August 31, 2010. Late Enrollments/waivers with an effective date of September 1, 2010 will be accepted until September 15, 2010.
	Police Academy Students:
	Open Enrollment for Police Academy students ceases July 31, 2010. Late Enrollments with an effective date of August 1, 2010 will be accepted until August 15, 2010.
	All Other Students (Voluntary Students):
	Open Enrollment for Voluntary students ceases August 31, 2010. Late Enrollments with an effective date of September 1, 2010 will be accepted until September 15, 2010.
Refund Policy:
No refunds for premiums will be authorized without confirmation from the College of a student’s termination and/or withdrawal from full-time status.
Termination of insurance is the first of the month following receipt of notification from the College on a student’s withdrawal from classes.
Qualifying Event:
If at any time a student comes of age and is no longer eligible to be insured as a dependant under their parent's plan, they may enroll in the Mercyhurst College Student Health Plan. The student must enroll within 30 days of a qualifying event. Other qualifying events include: Loss of coverage, marriage, divorce, birth of a child, etc. Documentation of the qualifying event must be provided.
